Nearly one-third of all health care costs in the United States are for hospitalizations. 

  • 37% of all Medicare spending is for inpatient care.
  • 18% of all Medicare patients are readmitted within 30 days of discharge.

Not only are these readmission very costly, they are also detrimental to the patient and negatively impacting the post-discharge agencies metrics.

While some readmissions are necessary, many may be prevented by coordinating post-discharge care. 

RIS provides immediate assessment of patients and assists in execution of patient transfers—to skilled nursing, long-term care or readmission for hospitalization is necessary. When readmission is determined to be necessary, the patient is directly admitted bypassing the ER.
